Please also include anything that may be beneficial for the coaches that may make the experience better for the child. Parental/Guardian consent

By ticking this box you acknowledge if the participant is under the age of 16 a legal parent of guardian must give consent for the child to participate in the activity, subject to the following: The child is medically fit to participate in the activity, the child may receive emergency first aid treatment, emergency medical, surgical, dental treatment as considered necessary by the medical authorities present, and the understanding that if the child's behavior jeopardises their own safety or the safety of others they may be removed from the activity. P4-P7 Camp visit the cinema during the week and will be expected to walk to and from Oban Pheonix Cinema with the coaches.

P4-P7 Holiday Camp Only

I can confirm my child is over 8 and can swim 2 lengths of a 25 metre pool unaided in compliance with our Pool Admission Policy. Anyone that is unable to do this will be allowed in the small pool with a swimming teacher providing fun games.
